Insurance:  We currently take Cigna, Blue Cross Blue Shield, Optum, United, Kaiser, Beacon, Aetna, UMR, Tricare, Medicaid and Medicare.

Services may be covered in full or in part by your health insurance or employee benefit plan. Mental Health and Substance Use Disorder Coverage Parity laws require most health plans to apply similar rules to mental health benefits as they do for medical/surgical benefits. Please check your coverage carefully by asking the following questions of your insurance provider:

  • Do I have mental health insurance benefits?
  • What is my deductible and has it been met?
  • How many sessions per year does my health insurance cover?
  • What is the coverage amount per therapy session?
  • Is approval required from my primary care physician?

Sliding Scale: Effective May 20, 2026, Monumental Therapy Group is committed to increasing access to mental health services. A Sliding Fee Discount Program may be available for eligible patients based on household income and family size. Eligibility is determined through a confidential application process and supporting documentation. Reduced- fee services are subject to provider availability and clinical appropriateness. Applicants may be asked to provide proof of income or public assistance eligibility, such as a recent pay stub, tax return, or Medicaid/SNAP approval letter. Documentation can be submitted through the client portal or by contacting our office for secure submission instructions. • Reduced-fee appointments are subject to provider availability and clinical appropriateness.

• Sliding fee approvals may be reviewed periodically and may require updated documentation.

• Discounted services may be limited based on practice capacity.

• Assignment to a specific clinician depends on scheduling availability, clinical needs, and provider qualifications.

• Intern clinicians and associate-level clinicians may offer additional reduced-cost services when appropriate.

Clients remain financially responsible for any outstanding balance accrued prior to approval of a reduced-fee arrangement, unless otherwise approved in writing by the practice. Participation in the sliding scale program does not automatically waive previous charges, missed appointment fees, late cancellation fees, deductibles, copays, or balances already owed. Clients with outstanding balances may be asked to establish a payment plan in order to continue services.

Patients will not be denied access to services solely due to inability to pay.  


Please note the cancellation policy. If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session.
